PATIENT SERVICES

  • Non-urgent, Primary healthcare for adults, ages 18 – 65
  • Dental – extractions only

    We do not have the resources for mental health or pain management.

COST TO PATIENT

Patients are encouraged to contribute to the cost of their care.

ELIGIBILITY

To qualify for care, annual family income must not exceed two and a half times the poverty level. The US Health and Human Resources 2019 guideline for poverty level is $12,140 for a single person and $25,100 for a family of four.

Eligibility interviews begin at 4:00. You must appear in person and bring the following documentation:

1. Photo Identification — one of the following:

  • Texas Driver’s License
  • Texas Identification card (current or expired)
  • Passport or Visa
  • U.S. Immigration documents
  • ID from another country

2. Proof of Income

All family members in the household who earn wages must provide income verification in order to determine the eligibility of each member of the household. Bring as many of these documents as possible:

  • Last 2 paycheck stubs
  • Welfare benefit documents (TANF and Food Stamps)
  • SSI or Social Security certification documents
  • Unemployment documentation
  • Child support
  • Most recent tax return

3. Proof of address

One of the following showing residence in the last month:

  • lease
  • house payment
  • utility bill
  • 2 pieces of valid mail in your name
